Your name and date of birth need to match both your USI record and your photo ID.

If your existing details have already been checked and verified against the USI database and match your photo ID then we cannot make any changes without you first changing your details with the USI database and providing us with new matching photo ID.

Once you have changed your details with the USI office please send us a support ticket here and provide us with new photo ID so we can make any required changes to your account.
