In order to issue your Interim Certificate (NSW) we must have the following from you:
- A signed and correctly witnessed Statutory Declaration
Attached is a copy of the statutory declaration form you will need to print and instructions for completing it correctly. If you are coming from New Zealand for a trade show please use the attached NZ form (students from NZ residing in Australia need to use the Australian form).
After reading the statutory declaration take it to an approved officer, along with your photo ID. As the approved witness does not need to know you personally they have to sight your photo ID.
DO NOT SIGN THE STATUTORY DECLARATION UNTIL YOU ARE IN FRONT OF YOUR APPROVED WITNESS.
A list of approved officers is also contained in this document. (In Australia, a local pharmacist, police station or post office may be the easiest to access. In NZ an officer of the crown or local authority may be the easiest).
Please scan or photograph a copy of the signed statutory declaration and upload the scan for verification through your student portal. Alternatively, you can email it to firstname.lastname@example.org
